This was a medium sized epidermal cyst that was a bit harder to remove than I would have thought initially. As a result, it was necessary to rupture the cyst capsule, to reduce the overall size of the lesion and also to increase the spacing within which I was working in. Also, because of the tight space, the lining of the incision was nicked a few times and needed to be cleaned up at the end. Minor amounts will heal without consequence but if the margin is jagged at all, that should be cleaned up to ensure good apposition. This is certainly not something you’ll see in OR settings or when you have extra assistants with retractors in place, but independent practitioners working in their offices or in clinic/ER settings will have to be able to deal with this appropriately. It really shouldn’t impact on overall healing as long as it is recognized and corrected.
