The distinction between an external and internal chalazion can be difficult to make. In general, I usually approach most chalazia through an internal approach, but for some that seem to be all external, I will approach it through the skin. The disadvantage of this is that you could get some unfavorable scaring. If I approach it externally, I will make a horizontal incision, with the thought that it will heal better. I do not place any sutures to repair it.
